You should have received a 1095-C form from Vanderbilt University Medical Center, delivered to the home address listed in Workday. You can also find a copy of this form in Workday by clicking on Benefits and Pay, then Benefits, then ACA form or by searching the term "ACA form." The form will be available in Workday, beginning March 1. This form, which is for the 2023 tax year, outlines each person in your household covered by VUMC’s health plan during the year, and which months they were covered.
